The After-Discharge Checklist for Families
Discharge day moves fast. This checklist helps you leave the ward knowing exactly what happens next at home.

Before you leave the ward
- Confirm exactly what care continues at home, and how often
- Collect the discharge summary and any prescriptions
- Ask whether an AIC subsidy referral has been started
- Find out what consumables you need at home
Arrange the first home visit
If a nurse needs to continue catheter, tube, wound or injection care, line up the first visit for the day of or day after discharge so there's no gap. Private home nursing can usually start straight away if you can't wait for a subsidised place.
Set up the home
Prepare a clean, well-lit space, and put supplies and medications where they're easy to find. A little setup makes the first days home far less stressful.
Know who to call
Make sure you have a number to call if something looks wrong — and that you know the red flags to watch for. For severe symptoms, call 995 or go to the nearest A&E.
