What it costs

from S$90 per visit for the nurse's time. Consumables are billed separately at cost — no markup. After-hours and urgent same-day visits add 40% to the nursing fee (evenings, weekends, public holidays); offshore-zone visits are quoted before you book. What happens during the visit

• The nurse goes through every current medicine and the doctor's latest list.
• Medicines are sorted into a labelled weekly pack by day and time.
• The nurse checks for duplicates, clashes, and doses that look wrong, and flags them.
• The family is shown how the pack works and what each medicine is for.
• A written visit summary is left for the patient's own doctor.

Who needs this

People on many medications — especially the elderly or those recently discharged with a changed medication list — and families worried that doses are being missed, doubled, or muddled.

How often: Often weekly for pill-pack setup, or a one-off review after a hospital stay or a change in medicines.

When to call a doctor

• New confusion, drowsiness, falls, or a rash after a medication change
• Two medicines that look like they do the same thing, or a dose that seems too high
• Doses regularly missed or doubled despite the pack
