Arranging Care Before Hospital Discharge

Plan before, not after

The riskiest stretch of a recovery is often the first week at home, when new medications, wounds or tubes meet a house that isn't set up for them. The families who cope best are the ones who arrange the care while their relative is still in the ward — because once discharge happens, you're solving problems in real time instead of ahead of them.

Ask the ward team early

Every hospital ward has people whose job is discharge planning — the medical social worker and care coordinators. Ask them, early, what care your relative will need at home and what support you may qualify for. Crucially, ask them to start any subsidy or home-care assessment before discharge, because that assessment is far easier to arrange from a hospital bed than from home.

What to line up

• Home nursing for any clinical needs — wounds, a catheter, injections, a feeding tube, or monitoring
• Daily help — a caregiver, helper, or family rota — for washing, meals and mobility
• Equipment the home will need: a hospital bed, wheelchair, commode, or pressure-relief mattress
• Transport home, and to follow-up appointments
• A clear, reconciled medication list — what changed in hospital, and what to take now
• The follow-up appointments themselves, booked before you leave

Sort the money question early too

Ask the ward to help start a means-test or Home Caregiving Grant assessment before discharge — it's the same paperwork whether you begin now or later, but starting early means support is in motion by the time the bills arrive. Get the home ready

A little setup makes the first days home far calmer: a clean, well-lit space, supplies and medicines where they're easy to find, and hazards cleared.
